Hemolytic uremic syndrome (HUS) is characterized by MAHA and acute kidney injury, and commonly thrombocytopenia and neurologic dysfunction. HUS is a separate entity from TTP based on ADAMTS13 enzyme activity. Shiga toxin–producing Escherichia coli HUS (STEC-HUS, also known as typical HUS) accounts for 90% of HUS cases and is caused by STEC organisms such as O157:H7 and Shigella dysenteriae.12 It predominantly affects children. STEC-HUS has a classic prodrome of abdominal pain with diarrhea, often preceding MAHA, acute kidney injury, and thrombocytopenia by five to 10 days. STEC urinary tract infections can cause a diarrhea-negative STEC-HUS.13 Streptococcus pneumoniae, human immunodeficiency virus, and influenza have also been associated with HUS in rare cases, which presents without the classic prodrome. There is an atypical HUS that also does not have the prodrome, is caused by complement dysregulation and not infection, and can be hereditary. Exacerbations can be triggered by an upper respiratory tract infection.14Cold AIHA involves IgM antibodies (cold agglutinin titers) that react with polysaccharide antigens on the RBC surface at low temperatures and then cause lysis on rewarming by complement fixation and intravascular hemolysis. Development of these antibodies is associated with infectious or malignant processes. Mycoplasmal pneumonia and mononucleosis are the two most common processes. Treatment of patients who have cold AIHA typically involves supportive measures, avoidance of triggers, and underlying disease management.Hemolysis indicates reduced RBC lifespan due to destruction of RBC.  Red blood cells are destroyed when they are prematurely removed from the circulation by macrophages, which phagocytize the cells before their normal lifespan is up. This is called extravascular hemolysis and the phagocytosis by macrophages is occurring within the spleen, in particular, but also other organs such as the liver and bone marrow. Extravascular hemolysis (phagocytosis of RBC by macrophages) is always occurring in a hemolytic anemia. In a few animals with hemolytic anemia, their RBC may also rupture (“pop”) within blood vessels. This is called intravascular hemolysis but it does not occur in all cases of hemolytic anemia, only in some unfortunate patients with some causes of hemolytic anemia (e.g. oxidant injury, immune-mediated hemolytic anemia). Note that many animals have anemia due to multifactorial causes (e.g. hemorrhage and decreased bone marrow production from inflammatory disease). Although plasma exchange is superior, fresh frozen plasma infusion is beneficial and should be started if transfer to a plasma exchange–capable center is delayed.11Hemolysis is the destruction of RBCs. The clinical picture of the dog or cat with HA is nonspecific and may relate not only to hemolysis, but also to an underlying disease process. Although anemia due to defective or deficient RBC production is often insidious in onset, HA frequently results in an acute illness. As a result of the anemia and decrease in oxygen delivery to the tissues, owners may notice exercise intolerance, anorexia, general malaise, syncope or collapse, or pallor. On physical examination, pale mucus membranes are a characteristic finding and icterus is often noted. The Coombs test helps to distinguish autoimmune (positive Coombs test) from non-autoimmune anemias (negative Coombs test).
