  3. The most common method of repair is shown in Figures 49-2 and 49-3. A small transverse infraumbilical incision is made, usually placed in the redundant skin, which is inverted at the conclusion of the procedure, thereby burying the closed incision. The hernia sac is identified and dissected free from the dermis, underlying the umbilical cicatrix. Our technique includes excision of the sac to the fascial edges, although other authors have described a more limited excision or simple inversion of the sac through the fascial opening. Interrupted sutures of nonabsorbable or long-lasting absorbable sutures are placed and tied, closing the fascial defect in a transverse fashion. We leave the needle attached to the center fascia suture, which is then used to tack the underside of the umbilical cicatrix to the fascia. The skin incision is closed with an absorbable subcuticular suture, and a dressing is applied. Pressure dressings are generally unnecessary.16

It has been known for many years that umbilical hernias will close spontaneously, and it seems very safe to simply observe the hernia until age 3 to 4 years to allow closure to occur. Pressure dressings and other devices to keep the hernia reduced do not speed the resolution and may result in skin irritation and breakdown and are therefore not advisable. Prospective studies in both white and African-American populations have shown spontaneous resolution rates of 83% to 95% by 6 years of age.6-10 Another study has shown 50% of hernias still present at age 4 to 5 years will close by age 11 years.9 One study suggests that hernias with fascial defects greater than 1.5 cm are unlikely to close by age 6 years, whereas other series suggest that even large defects will spontaneously resolve without operation.8,11,12 Postpartum Compression. This operation (or simple fascial closure) is still performed frequently today by many surgeons. These repairs are effective and may be the preferred technique for small umbilical hernias with no tension after fascial approximation, but larger hernias have been shown to have a recurrence rate of up to 28%.55 The incidence rate of abdominal wall hernia is 20-40% in cirrhotic patients. A surgical approach was originally performed only if complication signs and symptoms occurred. Several recent studies have demonstrated the usefulness of elective surgery

Laparoscopic Repair of Incisional Hernia Maria B. ALBUJA-CRUZ, MD University of Colorado Department of Surgery-Grand Rounds Overview Definition Advantages of Laparoscopic Repair Disadvantages of Open Repair The operative closure of an umbilical hernia uses standard techniques, is generally straightforward, and can usually be completed as an outpatient procedure. Although a “minimally invasive” technique has been described using injection of a sclerosing agent,15 most of these abnormalities are repaired using more traditional suture closure of the defect. Methods used commonly in the adult, such as prosthetic placement, are almost never needed in the child.

Children's umbilical hernias often close on their own in the first two years of life, though some remain open into the fifth year or longer. Umbilical hernias that appear during adulthood are more likely to need surgical repair. The results of inguinal hernia repair in this patient population are similar to those for umbilical herniorrhaphy (Inagaki et al, 2009; Lawson et al, 2009; Patti et al, 2008). Outcomes after elective repair do not vary between cirrhotic and noncirrhotic patients, although outcomes after emergent repair of an incarcerated or strangulated inguinal hernia are significantly worse in patients with cirrhosis (Carbonell et al, 2005). It appears that cirrhotic patients may safely undergo inguinal hernia repair with mesh; local wound complications, if they do arise, are usually easily treated (Inagaki et al, 2009; Lawson et al, 2009). Moreover, quality of life improves after elective inguinal hernia repair in symptomatic patients with cirrhosis (Patti et al, 2008). In short, it is reasonable, and the data would suggest even advisable, to repair hernias electively in patients with cirrhosis. However, as with any elective operation, patients should be well compensated and ascites controlled. The strategy for treating umbilical hernia in patients with ascites has evolved considerably. Initial management consisted of nonoperative therapy secondary to fear of precipitating bleeding from esophageal varices. An umbilical hernia creates a soft swelling or bulge near the navel. It occurs when part of the intestine protrudes through the umbilical opening in the abdominal muscles. Umbilical hernias in children are usually painless.

After birth, closure of the umbilical ring is the result of complex interactions of lateral body wall folding in a medial direction, fusion of the rectus abdominis muscles into the linea alba, and umbilical orifice contraction aided by elastic fibers from the obliterated umbilical arteries. Fibrous proliferation of surrounding lateral connective tissue plates and mechanical stress from rectus muscle tension may also aid in closure. 3. The introduction of mesh prosthetics has appropriately had an impact on umbilical hernia repair. These tension-free repairs, which have been popularized for other ventral hernias, may have a role in umbilical hernia repair. The largest randomized controlled trial looking at mesh in umbilical hernia repair was completed in 2001 by Arroyo et al. This randomized controlled trial compared primary suture repair and mesh repair in 200 patients with umbilical hernias.56 The two patient groups were comparable with regard to age, sex, hernia defect size, and American Society of Anesthesiologists (ASA) class. Operative times and complications were not statistically different. The mean follow-up was 64 months. The major difference was the recurrence rate of 11% in the suture repair group versus 1% in the mesh repair group (P = .0015). A review of all randomized controlled trials and observational studies found no difference in complication rates associated with mesh use, and an odds ratio of 0.09 in favor of mesh in randomized controlled trials and 0.40 in observational studies support the use of mesh in reducing umbilical hernia recurrence.57

Advertising revenue supports our not-for-profit mission.During gestation, the umbilical cord passes through a small opening in the baby's abdominal muscles. The opening normally closes just after birth. If the muscles don't join together completely in the midline of the abdominal wall, an umbilical hernia may appear at birth or later in life.

There are many effective methods to repair umbilical hernias. Each patient must be evaluated individually, and one method of repair may not apply to all cases. Small primary umbilical defects in low-risk patients can probably be repaired with sutures alone and achieve acceptable results. As the defect size increases, particularly in obese patients or manual laborers, a mesh prosthetic should be considered. Whether the repair is better performed via an open or laparoscopic approach is controversial because prospective data are not available. Improvements in mesh prosthetics may continue to guide the ideal approach. The incidence of umbilical hernia in the general population varies with age, race, gestational age, and coexisting disorders. In the United States, the incidence in African-American children from birth to 1 year old ranges from 25% to 58%, whereas white children in the same age group have an incidence of 2% to 18.5%.1,2 Premature and low birth weight infants have a higher incidence than full-term infants, reported to be up to 75% in infants weighing 500 to 1500 g at birth.3 Infants with certain other conditions, such as Beckwith-Wiedemann syndrome, Hurler’s syndrome, various trisomy conditions (trisomy 13, 18, and 21), and congenital hypothyroidism, also have an increased incidence of umbilical hernia, as do children requiring peritoneal dialysis.4,5 54 ORIGINAL ARTICLE Comparative evaluation of Sublay versus Inlay meshplasty in incisional and ventral hernias Muhammad Ayub Jat, Muhammad Rafique Memon, Ghulam Haider Rind, Syed Qarib Abbas Shah Abstract: IP: tomach Intestinal Pylorus paring urgery Why a loop and new approach makes sense! Inguinal hernia repair A hernia is an abnormal protrusion on an organ through a weakness in the abdominal wall. The abdominal muscles are usually strong enough to keep your internal organs in place, when Marsman and colleagues (2007) looked at outcomes of elective UHR versus conservative management in patients with umbilical hernias, cirrhosis, and ascites. Seventeen patients with a median MELD score of 23 (range, 18 to 26) underwent elective UHR, and another 13 patients, also with a median MELD score of 23 (range, 18 to 27), were observed. In the elective UHR group, 3 patients (18%) had local wound complications, but there were no instances of hepatic decompensation or perioperative death. In contrast, in the group that was observed, complications developed in 10 patients (77%), including nine cases of incarceration and one spontaneous rupture with evisceration; these led to emergency UHR in 6 patients, and there were two perioperative deaths (15%). Several other studies have found increased complications of patients treated emergently and advocate for elective repair of suitable patients not expecting imminent liver transplantation (Choi et al, 2011; Eker et al, 2011).

The ideal technique for placement of a prosthetic during umbilical hernia repair remains debatable. Laparoscopic techniques have recently been proposed for umbilical hernias as well. The technical aspects are essentially the same as those applied to other ventral hernia defects. The laparoscopic approach takes longer to perform, tends to have fewer complications, and has no recurrences reported in a small retrospective series. Criticism of the laparoscopic approach includes the need for general anesthesia to establish pneumoperitoneum, and the increased length and cost of operating time. Conversely, placement of trocars around but not through the umbilicus has the potential to avoid the wound-related complications associated with an incision directly over the mesh.As a general rule the incidence of exomphalos in the first trimester is rare (1/300). A policy of karyotype evaluation for all cases would not significantly affect overall invasive procedure rates.77 The high association with chromosomal abnormalities may warrant a karyotype investigation though some authors argue that delaying the procedure to the second trimester is possible if there is normal NT, no liver involvement nor other associated abnormalities.90 The highest 5-year prevalence of umbilical hernia repairs was seen in males aged 60-70 years with a 5-year prevalence of 0.53 % (95 % CI 0.51-0.56 %) and the highest age-specific 5-year prevalence of epigastric Instant access to the full article PDF. 34,95 €. A retrospective review of the Veterans' Affairs National Surgical Quality Improvement Program (VA NSQIP) database, analyzed the results of UHR in 127 cirrhotic and 1294 noncirrhotic patients (Gray et al, 2008). It determined that UHR performed electively in patients with cirrhosis was associated with an outcome similar to that in patients without cirrhosis. When UHR was performed emergently, however, patients with cirrhosis had significantly worse outcomes than those without. One drawback of this study was that the authors were not able to classify the degree of hepatic impairment among the patients with cirrhosis. Start studying Hernia Umbilical. Elective repair of an umbilical hernia in patients with ascites should be performed after proper optimization of the patient.Historically, mortality rates for umbilical hernia repair (UHR) in patients with cirrhosis were prohibitively high, and the prevailing opinion among surgeons was that uncomplicated hernias should be left untreated (Baron, 1960; O'Hara et al, 1975). UHR was only undertaken when complications, such as incarceration, strangulation, skin breakdown or impending rupture, or overt rupture, necessitated immediate intervention. In the present era, however, outcomes appear to be significantly improved for elective and even complicated UHR in patients with end-stage liver disease. Most series published in the literature since the early 1980s have reported little to no perioperative deaths and relatively low morbidity, although most of them have been relatively small retrospective studies (Belghiti et al, 1990; de la Pena et al, 2000; Ozden et al, 1998; Runyon & Juler 1985).
